Fiber is the second target after protein that a GLP-1 quietly makes harder to hit — and most of the advice about it online is written by companies selling fiber. Protein gets most of the attention on a GLP-1, and it should. Fiber is the second thing that quietly gets harder — you're eating less food overall, and fiber comes packaged in exactly the bulky, filling plant foods that are hardest to finish on a small appetite.
There's a measured version of this. According to PubMed, a small cross-sectional study of 69 people on a GLP-1 for at least a month found average fiber intake of about 14.5 g/day (Johnson et al., Frontiers in Nutrition, 2025, DOI ↗ (external link), via PubMed). It's a small, self-reported, industry-affiliated sample, so treat it as a signal rather than a population estimate — but it points the same direction as US data generally, where mean intake is around 17 g/day and only about 5% of people meet the recommended level.
The catch is that almost everything written about fiber for GLP-1 users is published by companies selling fiber supplements. That content is not all wrong, but it consistently overstates how much you need and understates how much the type matters. Here's the version with the evidence attached.
How much fiber do you actually need?
The Adequate Intake is set by the Institute of Medicine and endorsed by the Academy of Nutrition and Dietetics. It has two forms, and the difference between them matters a lot on a GLP-1 (Dahl & Stewart, J Acad Nutr Diet, 2015, DOI ↗ (external link), via PubMed):
The standard Adequate Intake is 25 g/day for women and 38 g/day for men under 50 (21 g and 30 g over 50) — but it's also expressed as 14 g per 1,000 calories, and those two versions diverge once a GLP-1 cuts your intake. At around 1,300 calories a day, the ratio implies roughly 18 g rather than 38 g. No health authority has issued GLP-1-specific fiber guidance, so this isn't settled, but the ratio is the more coherent yardstick while you're eating substantially less. Track a few typical days to find your own number rather than guessing.
Do men and women need different amounts of fiber?
Yes, under the standard targets: 38 g/day for men and 25 g/day for women under 50, dropping to 30 g and 21 g over 50. That difference mostly reflects the fact that men typically eat more calories — the underlying recommendation is the same for both, 14 g of fiber per 1,000 calories. So if a GLP-1 has reduced your intake a lot, the calorie-based figure is more informative than the sex-based one.
Which fiber supplement is best for GLP-1 constipation?
Psyllium. It's a gel-forming fiber that resists fermentation, so it's still present in the large bowel to hold water in the stool, and it has randomized trial evidence behind it for chronic constipation. Prebiotic fibers like inulin, fructooligosaccharides, and wheat dextrin do not produce a laxative effect — they're fermented away before they can — and wheat dextrin and finely ground wheat bran can actually be constipating. Check the ingredient, not the marketing: Metamucil's fiber is psyllium, Benefiber's is wheat dextrin.

Those two framings agree at ordinary intakes. A man eating 2,700 calories hits 38 g either way. They come apart badly once a GLP-1 has cut your intake.
If you're eating around 1,300 calories — a realistic figure on these medications — the ratio implies roughly 18 g of fiber a day, not 38. Chasing the absolute number at that intake would mean eating fiber at nearly double the density the recommendation was built on, from a food budget that also has to deliver enough protein to protect your muscle.
Two honest caveats. The ratio was derived for protection against coronary heart disease at normal intakes, not validated for people deliberately eating far less. And no health authority has issued GLP-1-specific fiber guidance — this tension is real, not resolved. What follows from it is modest and defensible: the standard adult targets are probably the wrong yardstick while your intake is low, the 14 g/1,000 kcal ratio is the more coherent one, and 14.5 g/day is a smaller shortfall against that ratio than the marketing suggests. The three things that actually determine what fiber does
"Soluble versus insoluble" is the split everyone learns, and it's a poor predictor of what a fiber will do for you. A review in the Academy's own journal set out to correct exactly this misconception, and the properties that actually matter are viscosity, fermentability, and particle coarseness (McRorie & McKeown, J Acad Nutr Diet, 2017, DOI ↗ (external link), via PubMed).
Viscosity — does it form a gel? In the small intestine, the benefits for cholesterol and blood sugar track viscosity closely. Gel-forming fibers — psyllium, beta-glucan (oats, barley), raw guar gum — deliver them. Non-viscous soluble fibers like inulin, fructooligosaccharides, and wheat dextrin do not, despite being just as "soluble."
Fermentability — does it survive the trip? For a laxative effect, the fiber has to still be there when it reaches the large bowel. Fibers your gut bacteria ferment away can't hold water in the stool, because they're gone.
Coarseness — is it big enough to matter? Large, coarse insoluble particles (wheat bran) work mechanically. Finely ground versions of the same fiber lose the effect.
Put together, there are only two mechanisms that actually relieve constipation: coarse insoluble particles that stimulate the gut wall, and gel-forming soluble fiber that resists dehydration. Both require the fiber to resist fermentation.
That has a blunt practical consequence, stated directly in that review: soluble fermentable fibers — inulin, fructooligosaccharides, wheat dextrin — do not produce a laxative effect, and some fibers can actually be constipating, wheat dextrin and finely ground wheat bran among them.
Matching the fiber to the problem you actually have
This is the single most useful thing on this page. If constipation is your problem — and it is for a lot of people on these medications — a prebiotic inulin product is not the tool, however healthy its marketing sounds. Psyllium is the one with reproducible trial evidence behind it for this specific job.
That evidence is decent. A randomized double-blind trial found psyllium superior to docusate sodium for chronic constipation, softening stool and increasing stool water content (McRorie et al., Aliment Pharmacol Ther, 1998, DOI ↗ (external link), via PubMed). A 2025 review of dietary management for chronic constipation puts psyllium among the strategies actually supported by randomized evidence — and notes, pointedly, that some things clinical guidelines routinely recommend are not well supported, naming insoluble fiber supplements specifically (Dimidi, Proc Nutr Soc, 2025, DOI ↗ (external link), via PubMed).
Powder, capsules, and gummies: do the label math
This is where most of the money gets wasted, and it's simple arithmetic.
Fiber works at gram-scale doses. Psyllium studies generally use something in the range of 5–10 g. A capsule physically cannot hold much — typically 0.4–0.5 g of fiber each — so matching a single powder dose means swallowing roughly seven to ten capsules, several times a day if you're dosing more than once.
Gummies are worse on this axis. A typical fiber gummy carries 2–3 g of inulin or chicory root fiber per serving — a fiber that, per the physics above, doesn't produce a laxative effect at any dose. Low dose and the wrong mechanism.

Numbers vary by product, and published figures disagree — so don't take the ones above as gospel for what's in your cupboard. Do this instead: find "Dietary Fiber" in grams on the Supplement Facts panel, note the serving size it refers to, and divide. That tells you what one capsule or one scoop actually delivers, and how many you'd need. Ingredient name matters as much as the number: psyllium husk and wheat dextrin are not interchangeable, whatever the front of the box says. Metamucil's fiber is psyllium; Benefiber's is wheat dextrin.
How to start without making things worse
Go up slowly. Adding a lot of fiber at once reliably causes gas, bloating, and cramping — unpleasant generally, and easy to mistake for a medication side effect while you're titrating a dose.
Fluid is not optional. Gel-forming fiber works by holding water. Taken without enough of it, bulk-forming fiber can make constipation worse rather than better. If your fluid intake has dropped along with your appetite, fix that first — see hydration on a GLP-1 and, if you've been losing fluids, electrolytes.
Separate it from your other medications. Fiber can interfere with the absorption of other drugs, so take it a couple of hours apart from them and ask your pharmacist about anything you depend on precisely.
Constipation that doesn't budge is a conversation, not a supplement problem. More than a week despite fiber, fluid, and movement — or significant pain, bloating, or nausea alongside it — is worth raising with your prescriber, as covered in the constipation guide.
One caution I can't give you evidence for: it's sometimes suggested that bulk-forming fiber is risky when a GLP-1 has slowed gastric emptying. It's a mechanistically reasonable worry, but I could find no published evidence establishing that risk, so it isn't a reason to avoid fiber. The fluid point above is the one that's actually supported.
What the marketing gets wrong
Oddly specific targets. Figures like "28–34 g/day, 60% soluble" circulate widely and match no dietary reference standard. Precision is not the same as evidence.
Treating fiber as a GLP-1 substitute. Fermentable fiber does stimulate your own GLP-1 release, and that's real — but your body clears its own GLP-1 in minutes while the medications hold for a week. Selling "soluble" as a synonym for "effective." Inulin is soluble and won't relieve constipation. Viscosity and fermentability decide what a fiber does; solubility alone doesn't.
Food first, as always
Supplements close gaps; they don't replace the food. Legumes, vegetables, fruit with the skin, whole grains, nuts, and seeds bring fiber along with protein, micronutrients, and water. Small portions still count, and a few high-fiber staples in reach beats a perfect plan you can't eat. Judge your fiber target against your actual calorie intake rather than the standard adult number, aim at food first, and if you add a supplement, pick it by mechanism rather than marketing: psyllium for constipation, gel-forming fiber for blood sugar and cholesterol, prebiotic fiber only for what prebiotics actually do. Check the grams on the label, increase slowly, and drink the water.
