GLP-1 medications aren't sex-neutral. They tend to work harder in women, come with more side effects in women, and are even absorbed differently between the sexes — and some of that traces to estrogen and testosterone. Here's what the evidence actually shows, and what it means for what you should expect.
Women tend to lose more weight
Across GLP-1 analogs, women lose more weight than men on average. In the semaglutide STEP program the gap was consistent: 14.0% vs 8.0% (STEP 1), 7.5% vs 4.6% (STEP 2), and 16.2% vs 9.3% (STEP 4), women versus men respectively (Börchers & Skibicka, Endocrinology, 2024, DOI (external link), via PubMed).
The obvious guess — women are smaller, so a standard dose goes further — isn't the whole story. At the same body weight, women had about 32% higher drug exposure than men on liraglutide, and the review notes that even women who weighed more than the men still lost more. So it's not simply body size; the drug appears to do more per milligram in women.
Side effects hit women harder
The flip side of a stronger response is worse tolerability. Women report and discontinue for side effects considerably more often:
- Nearly 50% more women than men discontinued treatment overall.
- Twice as many women stopped specifically because of gastrointestinal side effects.
- Adverse-effect reports ran 28% higher in women for liraglutide and 41% higher for dulaglutide (Börchers & Skibicka, Endocrinology, 2024, DOI (external link), via PubMed).

Blood sugar: a smaller, less consistent gap
For blood-sugar control (HbA1c), the sex difference is less consistent than it is for weight — though where a difference shows up, it again tends to favor women. Notably, older postmenopausal women don't show the pronounced response that younger women do, one of several clues that estrogen is part of the mechanism (Börchers & Skibicka, Endocrinology, 2024, DOI (external link), via PubMed).
Why sex matters here: estrogen and testosterone
The differences aren't random — sex hormones interact directly with the GLP-1 system:
- Estrogen amplifies GLP-1's effect on appetite. In lab studies, estrogen enhances GLP-1's action on food-motivated behavior, removing estrogen (ovariectomy in animals) blunts the response, and a designed GLP-1–estrogen combination produced synergistic weight loss. Women also secrete more of their own GLP-1 in the luteal phase of the menstrual cycle than the follicular phase. Together these help explain both the bigger effect in women and why it fades after menopause.

One more social difference: uptake skews female, with women making up over 75% of the youngest (18–25) patients taking these medications.
The questions each sex should be asking are different, too
Beyond how the drug behaves, men and women face genuinely different considerations on a GLP-1:

What this means for you
A few honest caveats. These are averages from groups, not predictions about any one person — plenty of men are strong responders and plenty of women tolerate the medication easily. Standard doses are not sex-adjusted on the label, so this isn't a reason to change your dose on your own; it's context for setting expectations and for advocating on side effects. The universal advice doesn't change by sex either: protect muscle with resistance training and adequate protein while you lose weight. Where sex does usefully change things is anticipation — women may want to plan for a rockier tolerability curve and a stronger weight response, and both men and women benefit from knowing the hormone questions that are specifically theirs.
The bottom line
Men and women get real, measurable differences from the same GLP-1: women tend to lose more weight and have higher drug exposure but also more side effects and discontinuations, while the blood-sugar gap is smaller and fades after menopause — patterns that estrogen and testosterone help explain. None of it rewrites the dose on the label, but it does mean your experience is partly predictable from a variable no one used to account for, and it's worth bringing into the conversation with your prescriber.
