Stretching your GLP-1 dose past 7 days: what drug levels and early studies actually show
Some people space their weekly GLP-1 to every 10 or 14 days to save money or ease off. Here's what that does to drug levels, what the two small studies found, why that isn't proof yet, and the risks worth planning around with a prescriber.

Semaglutide (Wegovy, Ozempic) and tirzepatide (Zepbound, Mounjaro) are approved as once-weekly injections. In practice, some people stretch that to every 10 days, every two weeks, or longer. Usually the reason is cost or supply, sometimes side effects, and often a hope of using less medication once the weight has come off. The question underneath is a fair one: if the goal is maintenance rather than more loss, do you need the full weekly dose?
The honest answer right now is "possibly, for some people, but nobody has tested it properly." This guide walks through what spacing does to the amount of drug in your body, what the early evidence found, and what to plan for if you and your prescriber decide to try it.
That slow clearance is why a missed dose isn't an emergency, and it's also why spacing doses out doesn't drop you off a cliff. But it does change your levels in two ways.
Your average exposure drops in proportion to the gap. Averaged over time, the amount of drug in your system depends on how much you take and how often. Taking the same dose every 10 days gives you roughly 70% of the weekly average. Every 14 days gives you about half. That arithmetic holds whatever the half-life is.
Can I take my GLP-1 every 10 days or every two weeks instead of weekly?
It's off-label, and there are no randomized trials testing it. Spacing doses lowers your average drug exposure in proportion to the gap: about 70% of the weekly level at every 10 days and about half at every 14 days. A 30-person case series found that people who switched to mostly every-other-week dosing after reaching a plateau kept their weight off for about 36 weeks, which is promising but far from proof. Make any change with your prescriber and agree in advance on when to go back to weekly.
Does tirzepatide wear off faster than semaglutide if I space out doses?
Yes, at the tail end of each gap. Tirzepatide's half-life is about 5 days and semaglutide's about 7, so 14 days after a dose roughly 14% of a tirzepatide dose remains versus about 25% of a semaglutide dose. These are simple estimates, not measured levels, but they explain why appetite can return before the next shot on a longer schedule.
Is stretching the interval the same as microdosing?
Not quite. Stretching keeps the same dose and takes it less often, while microdosing usually means taking a much smaller amount than labeled. Both lower total exposure and neither has randomized trial support. Stretching with a prefilled pen at least keeps the dose itself accurate; custom doses drawn from compounded vials add measurement-error risk.
Is a lower weekly dose better than a longer gap between doses?
A lower weekly maintenance dose currently has stronger evidence. The randomized SURMOUNT-MAINTAIN trial tested a reduced tirzepatide dose after weight loss, while stretched intervals have only a small case series and a two-patient report behind them. That doesn't mean a longer gap can't work for some people. It means one option has been tested properly and the other hasn't yet.

These are simple single-dose estimates, not measurements, and your real levels depend on your weight, how long you've been on the drug, and your dose. The point is the direction: on a two-week schedule, the second week runs on much less medication than the first. Many people describe that as appetite and "food noise" creeping back before the next shot, which is exactly what the numbers predict.
What the evidence actually shows
There are no randomized trials of stretched dosing. What exists is small and early.
A 30-person case series (2026). Doctors at Scripps Clinic reviewed patients whose weight had plateaued on weekly semaglutide or tirzepatide (less than 5% change over three months) and who were then offered every-other-week dosing at their current dose. Of 38 who were interested, 4 never actually switched and 4 went back to weekly dosing because weight was creeping up. The 30 who stayed on the reduced schedule were followed for an average of 36 weeks. Most injected every two weeks or longer, up to six weeks, and six injected every 10 to 14 days. Their average weight went from 74.1 kg at plateau to 72.4 kg, so they held their loss and shed a little more. Blood sugar, triglycerides, and blood pressure gains held too (Wong et al., Obesity, 2026, DOI ↗ (external link), via PubMed).
That's genuinely encouraging, and it's also a very specific group. People only switched after a plateau, most had reached a BMI around 25, only two had class 2 or 3 obesity, 26 of 30 were White, and nobody was randomized or compared with a control group. People motivated enough to try de-escalating may also be doing more of everything else. The authors say plainly that larger randomized trials are needed. Body composition was measured with bioimpedance, which has real limits. One author is a trial investigator for both drug makers.
Two patients and a computer model (2025). A separate report described two people who kept most of their weight loss while dosing less often, paired with a pharmacokinetic model of "virtual patients" suggesting that most weight loss could persist with gaps of two, three, or perhaps four weeks. The authors call it a hypothesis to test, not a protocol (Wu et al., Obesity, 2025, DOI ↗ (external link), via PubMed).
Where it fits in the bigger picture. A 2026 review argued that long-term care shouldn't be a binary choice between full dose forever and stopping. It lists reduced-frequency dosing as one option alongside a lower dose, switching to an oral form, and structured lifestyle support, while stressing that none of these has been prospectively validated (van Bruggen et al., Obesity Pillars, 2026, DOI ↗ (external link), via PubMed). The closest thing to a proper trial is SURMOUNT-MAINTAIN, which tested a lower weekly dose rather than a longer gap. The risk you're managing: less drug can drift toward no drug
The reason to be careful is what happens when these medications stop entirely. In the STEP 4 trial, people who switched from semaglutide to placebo gained back 6.9% of their body weight over the next 48 weeks, while those who continued lost another 7.9% (Rubino et al., JAMA, 2021, DOI ↗ (external link), via PubMed). A 2026 analysis of 37 studies estimated that after stopping semaglutide or tirzepatide, weight comes back at about 0.8 kg a month, with a projected return to starting weight in about a year and a half (West et al., BMJ, 2026, DOI ↗ (external link), via PubMed).
Stretching isn't stopping. But every extra day between doses moves your exposure a little closer to zero, and nobody yet knows where the line is for any individual. That's why the early studies watched weight closely and let people return to weekly dosing when it climbed.
Practical risks to plan around
GI side effects when you "catch up." Tolerance to nausea and other GI effects is built during titration and fades with low drug levels. Long gaps followed by a full dose can bring side effects back. In one 2026 case report, a woman who had tolerated semaglutide for about a year took a five-week break, restarted at her full 2.4 mg dose, and was hospitalized for four days with severe vomiting, diarrhea, and electrolyte problems. The authors recommend restarting at a lower dose after long interruptions (Valizada & Zilbermint, Cureus, 2026, DOI ↗ (external link), via PubMed). If you take it for diabetes or heart protection. The case series participants mostly didn't have diabetes (average A1c before treatment was 5.6%), and none of the studies looked at cardiovascular outcomes. If blood sugar control or heart risk is a main reason you're on the medication, lower exposure could matter in ways that weight alone won't show.
If you and your prescriber want to try it
What the early studies did is a reasonable template to discuss, not a protocol to copy:
Start from a stable plateau, not in the middle of active loss.
Change one thing at a time: interval or dose, not both at once.
Pick a regain threshold in advance (for example, a set number of pounds or a clear return of food noise) that means going back to weekly dosing.
Keep the supports running: protein, resistance training, sleep, and regular check-ins. Those matter more once the medication is doing less.
Track more than the scale if you can: waist, how hungry you are late in the gap, and labs if you have metabolic conditions.
The bottom line
Spacing doses beyond 7 days lowers your average drug exposure in proportion to the gap and deepens the dip before each shot. Two small reports suggest some carefully selected people keep their results on an every-other-week schedule, which is promising and a long way from proven. What happens if I wait too long between doses?
Two things. Appetite and weight can drift up as drug levels fall, and the GI tolerance you built during titration fades, so resuming a full dose after a long gap can bring back nausea or worse. After extended gaps, prescribing information advises restarting the titration schedule rather than jumping back to your old dose, so check with your prescriber before restarting after several weeks off.
