Comparing the GLP-1 pills: Ozempic tablets (Rybelsus), the Wegovy pill, and Foundayo
There are now three GLP-1 pills in the US, and they aren't interchangeable: oral semaglutide for diabetes (Rybelsus, now sold as Ozempic tablets), oral semaglutide for weight (the Wegovy pill), and orforglipron (Foundayo). Here's how they compare on approval, how you take them, what the trials found, side effects, cost, and what you give up versus a weekly injection.

In its main trial, the Wegovy pill took off an average of 13.6% of body weight over 64 weeks. Foundayo's top dose took off 11.2% over 72 weeks. The pill most people already knew, Rybelsus, was never approved for weight loss at all. "The GLP-1 pill" now means three different products with different makers, approvals, and rules for how to swallow them, so it's worth knowing which one someone is talking about.

What's the difference between Rybelsus, the Ozempic pill, and the Wegovy pill?
All three are oral semaglutide from Novo Nordisk. Rybelsus and its reformulated US successor, Ozempic tablets (1.5, 4, and 9 mg, on sale since May 4, 2026), are approved for type 2 diabetes. The Wegovy pill (up to 25 mg) is approved for weight reduction and for lowering cardiovascular risk in people with heart disease and obesity or overweight. Doses aren't interchangeable milligram for milligram: Rybelsus 7 mg matches Ozempic tablets 4 mg, and 14 mg matches 9 mg.
Which GLP-1 pill causes the most weight loss?
No trial has compared the two weight-loss pills directly. In separate trials, the Wegovy pill produced 13.6% average weight loss at 64 weeks and Foundayo's top dose 11.2% at 72 weeks, each against about 2% on placebo. A Novo Nordisk-funded indirect comparison estimated about 3 percentage points more loss with the Wegovy pill. Neither pill has matched injectable tirzepatide, which produced 20.2% in its head-to-head trial with injectable semaglutide.
Do I have to take the Wegovy pill on an empty stomach?
Yes. The label says to take it in the morning on an empty stomach with no more than 4 oz of plain water, swallow it whole, and wait at least 30 minutes before eating, drinking, or taking other oral medicines. Only a small fraction of oral semaglutide is absorbed, and food in the stomach can block it almost entirely. Foundayo is the exception: it can be taken any time, with or without food.
Is Foundayo approved for type 2 diabetes?
Not in the US as of its current label (July 2026), which covers weight reduction and long-term maintenance only. Orforglipron has been studied in type 2 diabetes, and in one open-label trial it lowered HbA1c more than the Rybelsus doses of oral semaglutide, with more gastrointestinal side effects. For a diabetes pill, the approved US option is oral semaglutide, now sold as Ozempic tablets.

A note on names. In the US, Novo Nordisk has reformulated Rybelsus and is moving it to a new name: Ozempic tablets, in 1.5, 4, and 9 mg strengths, which went on sale on May 4, 2026. Rybelsus 7 mg matches Ozempic tablets 4 mg, and 14 mg matches 9 mg; the two aren't interchangeable milligram for milligram. Rybelsus patients are being told to keep taking their medication and to talk to their prescriber about switching. Novo has also filed for a 25 mg Ozempic tablet for diabetes, with a decision expected by the end of 2026. So "the Ozempic pill" is a diabetes drug, and "the Wegovy pill" is the weight-loss one, even though both are oral semaglutide.
How the semaglutide pills work, and why the rules are so strict
Semaglutide is a peptide, a small protein that stomach acid and digestive enzymes normally break down. The tablets get it through by pairing it with an absorption enhancer that works in the stomach. Even so, only a small fraction gets in. A pharmacology analysis estimated that under the recommended conditions, about 0.8% of the swallowed dose reaches the bloodstream, and that absorption rose with a longer wait before eating and fell when people drank more water with the tablet (Overgaard et al., Clinical Pharmacokinetics, 2021, DOI ↗ (external link)). That's why the pill doses are in the tens of milligrams while the weekly Wegovy injection is 2.4 mg.
The morning ritual comes straight from that research. In the study that set the rules, people who took the tablet 30 minutes after a meal had little or no measurable semaglutide in their blood. Water volume up to 120 mL (about 4 oz) didn't change absorption, but a longer fast after the dose increased it (Bækdal et al., Diabetes Therapy, 2021, DOI ↗ (external link)).
In practice, that means:
Take it first thing, on an empty stomach, with no more than 4 oz of plain water. No coffee, juice, or other drinks with it.
Swallow it whole. Don't split, crush, or chew it.
Wait at least 30 minutes before eating, drinking anything else, or taking other oral medicines.
If your mornings are unpredictable, or you take other pills that also need an empty stomach (levothyroxine is the common one), this is the main practical cost of the semaglutide pills. The labels also note that taking semaglutide tablets raised levothyroxine exposure by about 33% in a drug-interaction study, so thyroid levels may need checking.
How Foundayo differs
Orforglipron isn't a peptide. It's a small molecule, more like a conventional pill, so it doesn't need an absorption enhancer or an empty stomach. The label says to take it once a day, with or without food, at any time of day. Its half-life is about 29 to 49 hours, against about a week for semaglutide.
That convenience comes with a few different cautions on the label:
Birth control pills. Foundayo slows stomach emptying, which can affect how well other oral drugs are absorbed. The label advises anyone on oral hormonal contraceptives to switch to a non-oral method, or add a barrier method, for 30 days after starting and for 30 days after each dose increase. Drug interactions. Orforglipron is broken down by the liver enzyme CYP3A4. Medicines that strongly block or speed up that enzyme change how much Foundayo you're exposed to. The label caps the dose at 9 mg alongside a strong CYP3A4 inhibitor and says to avoid some combinations entirely (the HIV medicine ritonavir is its example). Give your prescriber and pharmacist your full medication list.
One more detail that confuses people: the trials tested orforglipron at 6, 12, and 36 mg of an earlier formulation. The marketed tablets are labeled 5.5, 9, and 17.2 mg, and the FDA label presents the trial results as those "equivalent dosages." When you read that the 36 mg dose produced 11.2% weight loss, that is the 17.2 mg Foundayo tablet.
What the trials found
Each pill was tested against placebo in different people over different lengths of time. The numbers below can't be lined up and ranked as if they came from one trial.
Wegovy pill (oral semaglutide 25 mg). OASIS 4 randomized 307 adults with obesity, or overweight plus a related condition, and without diabetes. At 64 weeks, weight fell by 13.6% on the pill versus 2.2% on placebo (Wharton et al., The New England Journal of Medicine, 2025, DOI ↗ (external link)). Novo Nordisk reports that people who kept taking the pill as directed lost about 17%. OASIS 4 was a small trial run at 22 sites and funded by Novo Nordisk.
Foundayo (orforglipron). ATTAIN-1 randomized 3,127 adults with obesity and without diabetes. At 72 weeks, weight fell by 7.5%, 8.4%, and 11.2% on the three doses, against 2.1% on placebo. At the top dose, 54.6% of people lost at least 10% of their weight (Wharton et al., The New England Journal of Medicine, 2025, DOI ↗ (external link)). In a second trial in people with obesity and type 2 diabetes, the FDA label reports 9.6% weight loss at the top dose against 2.5% on placebo. Eli Lilly funded both.
Rybelsus / Ozempic tablets (oral semaglutide for diabetes). In PIONEER 1, adults with type 2 diabetes managed by diet and exercise alone took 3, 7, or 14 mg for 26 weeks. HbA1c fell by 0.6, 0.9, and 1.1 percentage points more than on placebo; weight fell by 2.3 kg more than placebo at 14 mg (Aroda et al., Diabetes Care, 2019, DOI ↗ (external link)). These are diabetes doses, and weight loss is modest at them. In the SOUL trial of 9,650 people with type 2 diabetes and heart or kidney disease, oral semaglutide cut major cardiovascular events by 14% over about four years (12.0% vs 13.8%; hazard ratio 0.86) (McGuire et al., The New England Journal of Medicine, 2025, DOI ↗ (external link)).
Head to head: what little there is
In type 2 diabetes, orforglipron versus the Rybelsus doses. ACHIEVE-3 randomized 1,698 adults on metformin to orforglipron (12 or 36 mg) or oral semaglutide (7 or 14 mg) for 52 weeks. Both orforglipron doses lowered HbA1c more than both semaglutide doses: 1.91 points at orforglipron 36 mg against 1.47 at semaglutide 14 mg. Orforglipron also had more gastrointestinal side effects (58–59% vs 37–45%), more people stopping for side effects (9–10% vs 4–5%), and a larger rise in pulse (Rosenstock et al., The Lancet, 2026, DOI ↗ (external link)). Lilly funded it, it was open-label, and it did not include the 25 mg Wegovy dose.
For weight loss, Wegovy pill versus Foundayo. No trial has compared them directly. A Novo Nordisk-funded analysis statistically adjusted the OASIS 4 and ATTAIN-1 populations to compare them indirectly. It estimated about 3 percentage points more weight loss with oral semaglutide 25 mg and fewer people stopping for side effects (Michalak et al., Diabetes, Obesity and Metabolism, 2026, DOI ↗ (external link)). Treat this as a hint, not a verdict: it compares one small trial with one large one, and the manufacturer of one of the drugs paid for it.
Side effects
All three pills have the same class side-effect profile as the injections: mostly nausea, diarrhea, vomiting, constipation, and indigestion, worst during dose increases. The labels carry the same class warnings for pancreatitis, gallbladder disease, kidney injury from dehydration, and the boxed warning about thyroid C-cell tumors seen in rodents.
Wegovy pill: in OASIS 4, 74.0% on the pill reported a gastrointestinal event against 42.2% on placebo. The label reports 6.9% stopped for side effects against 5.9% on placebo.
Foundayo: across the two main weight trials, 8% stopped for side effects against 3% on placebo, rising to 10% at the top dose, mostly for GI reasons (FDA label).
Rybelsus / Ozempic tablets: in PIONEER 1, 2.3–7.4% of people stopped the trial drug, against 2.2% on placebo.
A pill means no needles or pens, and no refrigerator: all three tablets are stored at room temperature. That makes travel and supplies easier, and for some people it's the difference between starting treatment at all and not starting.
What you give up:
A weekly habit becomes a daily one. A weekly shot is one decision a week; a pill is seven.
The semaglutide pills come with the morning ritual.
You get less weight loss than the strongest injection. In the SURMOUNT-5 head-to-head trial, injectable tirzepatide (Zepbound) produced 20.2% weight loss over 72 weeks against 13.7% for injectable semaglutide (Aronne et al., The New England Journal of Medicine, 2025, DOI ↗ (external link)). No pill trial has reached the tirzepatide range. The Wegovy pill's result is in the same range as the Wegovy injection's. A Novo Nordisk modelling analysis found that 82.2% of people on the 25 mg pill in OASIS 4 reached semaglutide blood levels within the range seen with the 2.4 mg injection (Overgaard et al., Diabetes, Obesity and Metabolism, 2026, DOI ↗ (external link)).
If you've already lost weight on an injection and want to come off it, a pill is also a maintenance option. One trial found that people switching from injectable tirzepatide or semaglutide to Foundayo kept most of their loss for a year. See switching from a GLP-1 injection to a pill. Cost and access
Pricing and coverage change often, so check current terms before you rely on any number. Self-pay prices announced by the manufacturers start at $149 a month for the lowest dose of each pill: Novo Nordisk announced this for the Wegovy pill in December 2025 and for Ozempic tablets in May 2026, and Lilly announced the same starting price for Foundayo in April 2026. Higher doses cost more. All three have manufacturer savings cards for people with commercial insurance that covers them.
This is a starting point for the conversation with a prescriber, not a decision rule.
You have type 2 diabetes and want a pill: Ozempic tablets (formerly Rybelsus) is the approved option, with cardiovascular-outcome evidence from SOUL. Foundayo isn't approved for diabetes as of its current label, although it has been studied in diabetes.
You want the most weight loss a pill has shown and can do the morning routine: the Wegovy pill.
You want a pill without timing rules, or your mornings won't allow a 30-minute wait: Foundayo, keeping the contraception and drug-interaction cautions in mind.
You want the most weight loss possible, full stop: the injections, especially tirzepatide, still lead.
The bottom line
There are now three GLP-1 pills, and they aren't the same product. The two semaglutide pills demand an empty stomach and a 30-minute wait but have the longer track record, including cardiovascular data. Of the two weight-loss pills, the Wegovy pill posted the larger average loss in its trial, and the one indirect comparison also favors it, though no head-to-head trial exists. Foundayo is the easiest to take and has stronger blood-sugar results than the Rybelsus doses in diabetes, at the cost of somewhat more side effects in that trial and a few extra drug-interaction cautions. None has matched the weight loss seen with injectable tirzepatide. The best pill is the one you'll take correctly every day, at a price you can keep paying.

Does Foundayo affect birth control pills?
It can. The Foundayo label advises anyone using oral hormonal contraceptives to switch to a non-oral method, or add a barrier method, for 30 days after starting and for 30 days after each dose increase, because the drug slows stomach emptying and can affect absorption of other oral medicines. Contraceptives that aren't taken by mouth shouldn't be affected.
Is a GLP-1 pill as good as the weekly injection?
It depends on the comparison. The Wegovy pill's weight loss is in a similar range to the Wegovy injection's, and most people on it reach similar semaglutide blood levels. No pill has matched injectable tirzepatide. The pill trades the needle and refrigerator for a daily habit, and for the semaglutide pills a strict morning routine. The best choice is the one you can take correctly and afford over the long term.
